NHS Health Connector vacancy in West Mendip

Health Connections is recruiting for a new NHS Health Connector in the West Mendip area

* The full-time salary for the post is £25,420 / £13 an hour

* 30 – 37.5 hours per week (optional depending on the candidate’s preference)

* Evening working will be required once a week

* This is a permanent post

Health Connectors offer person-centred care and support, assisting people with a range of non-medical issues, and helping them to feel more connected in their communities.

They offer one-to-one support, signposting and health coaching, in the most appropriate way for the person. This can be on the phone, in GP practices, in the community or in people’s homes. Health Connectors also run groups and other projects in the local area such as Talking Cafes, healthy lifestyle programmes and pain management groups.

They help people to connect to their strengths and resources, build trust and respect, listen with presence and kindness, respect diversity and focus on what matters most to people. The Health Connector will work with people of all backgrounds, from 18-100. They will work as part of an innovative, supportive and passionate team.

This role is hosted by West Mendip Primary Care Network, which is a collaboration of 5 GP practices including Glastonbury Health Centre, Glastonbury Surgery, Vine Surgery, Wells City Practice and Wells Health Centre.

The person will have a particular focus on supporting patients and community engagement in the Glastonbury area. For some of the week they will work within a health and wellbeing hub in Street, alongside other Health Connectors, Care Co-ordinators, a GP, Clinical Nurse Lead, Occupational Therapists, Clinical Pharmacists and a Hospital Discharge Reviewer.
